A cockpit voice recorder (CVR), often referred to as a black box is a flight recorder used to record the audio environment in the flight deck of an aircraft for the purpose of investigation of accidents and incidents. This is typically achieved by recording the signals of the microphones and earphones of the pilots headsets and of an area microphone in the roof of the cockpit.

Since 1968 there have been 39 fatal incidents involving the Tu-154, most of which were caused either by factors unrelated to the aircraft or by its extensive use in demanding conditions. Such TU-154 crashed on final approach in thick fog on an airfield with no ILS on april-10-2010 near Smolensk. President Lech Kaczynski and other high-ranking officials were on board and died in the crash. The flight recorders of the Polish President’s plane registered the prayer of the pilot before the Tu-154 crashed. The Prosecutor General of Poland pledged to release all recordings of the pilots’ conversations. Two of the three black boxes from the presidential plane have already been deciphered and voice recordings of the third one are marred by noise and the information needs to be restored.

This black box comes from a TU-154 plane. There is a tape but it is not sure if there is recording on it.
